COVID-19 Vaccine Form

The Australian Government has commenced roll-out of the COVID-19 vaccination program. Selected GP Practices are now participating in PHase 1B of the national COVID-19 vaccine rollout. The government will be releasing vaccines in a highly controlled way, starting with people in the highest risk groups first.
